The temple's documented history centers on Velomangalath Nampi, a feudal administrator under the Kolathiri ruler centuries ago. According to local accounts, Nampi played a crucial role in the temple's establishment. A woman from the Kuppathi Tharavaad, married into the Mundayatt family of Annur, made annual offerings of dried rice to Poomala Bhagavathi at another shrine. When she arrived one year with rice to fulfill her vow—the year her son was born—her offering was rejected due to timing restrictions. Distressed, she approached Nampi with her husband and shared her sorrow. That night, signs suggested the goddess had arrived with the rice itself. Nampi subsequently oversaw the construction of a shrine in Annur and established the goddess there.
The six Poothwal Tharavaads—Kallidi, Chuvatt, Aanidi, Kupleri, Vellor, and Utthamantil—who participated in the temple's construction continue to hold the position of Koymakkar (principal custodians) at the shrine to this day.
